Understand how trepanation works. A hole is drilled into the skull to increase the volume of blood in the brain. This is thought to achieve a number of purposes, including the curing of migraines and depression, and the permanent alteration (and supposed expansion) of the patient's mental state. Some believe that this is accomplished by allowing bad or hindering spirits that are trapped inside the brain to exit through the hole in the skull.

Know the effects of trepanation. Some have likened it to standing on one's head. Increased flow of blood to the brain for a prolonged period of time causes a form of intoxication and altered perception. Trepanation increases the flow of blood to the brain continually.
